Refer your Patient
Please see below for instructions on how to refer your patient. We often offer second opinions on patients referred from other physicians or centres.
Patient or Referring Physician
You can send your referral for either clinic to the same email address: neurovascular@uhn.ca or fax: 416 603 6282. Please include the following:
- A referral letter
- Clinical notes
- All relevant diagnostic tests
Internal referrals need to be sent through EPIC
